Inside Totality Medispa
I first caught wind of Dr. Nicole Nadel at Totality Medispa when whispers began circulating through beauty insiders: she was the one behind Madelyn Cline’s lips. Yes — those lips. The kind that ignites social media frenzies, launches countless Pinterest boards and inspires millions to screenshot before calling their injector. The idea that a local South Carolina doctor could be responsible for one of the most recognizable features on the planet was…intriguing.

So I started digging. What I found was even more compelling. Eight years ago, Dr. Nadel and her husband opened the doors to Totality Medispa. Today, they’re not only running four thriving locations, but have two more in the works — an empire in motion.

Naturally, I had to know: what’s the secret? So I dove headfirst into Totality Medispa’s world, searching for the magic formula behind their meteoric success. The first thing that struck me was their social media. Their Instagram page wasn’t just active — it was a living, breathing portfolio. Yes, there were a few lighthearted posts sprinkled in, but the heart of their feed was something far more compelling: jaw-dropping results.

When I first met Dr. Nadel, I was struck by her face. There were no obvious signs that she’d ever had work done — no telltale overfilled cheeks, no duck lips, no frozen expression. And yet, as the owner of a medical spa for eight years, I knew she had probably enjoyed her fair share of treatments. The artistry was in the fact that you couldn’t tell. At that moment, I knew I was in the right place.

Dr. Nadel began my consultation, and even though I was there specifically for filler, she didn’t just stop there. She took the time to assess my overall skin health, making sure I was using the right skincare products and doing all the preventative things to protect my results long-term. I liked that approach — it felt comprehensive, thoughtful. When I told her so, she smiled and said, “That’s the reason it’s called Totality. We take a full-body approach to every patient, every time.”

Of course, I couldn’t resist. I asked her point-blank if she had, in fact, done Madelyn Cline’s lips. She couldn’t confirm it — confidentiality, of course.

The best part of the whole experience was that I left the office with a plan — a well-thought-out, custom-tailored plan just for me based on her evaluation. The list was long, but there was no pressure whatsoever to do it all at once. It was more of a roadmap, there for me when I was ready. I loved that approach to Totality — seeing the big picture while still giving the patient total control. I left impressed not only by her work but by her as a person.

Walking out of Totality Medispa, I realized this wasn’t just about filler, skincare, or even beauty — it was about trust. In an industry overflowing with trends, filters, and instant gratification, Dr. Nadel is playing the long game: natural, timeless results built on precision, artistry and genuine care.
